Judiciary Vaccination Registration for Priority Group A4.13 until 31 May 2021

OCA CIRCULAR NO. 64-2021

 

In line with the Vaccination Program of the Judiciary and in coordination with the National Task Force Against COVID-19 (NTF), the Department of Health, and the Local Government Unit (LGU) of the City of Manila, all judges and court personnel of the first and second level courts in the National Capital Judicial Region, including the judges and court personnel of the first and second level courts in the Third and Fourth Judicial Regions who reside in the National Capital Region, falling under Priority Group A4.131 and wish to be inoculated at the Supreme Court, the request for which is pending, shall accomplish the Vaccination Registration Form, which may be accessed via https://forms.office.com/r/zP4bBPXfUb or by scanning the QR code provided below, on or before 31 May 2021.

Those who registered under OCA Circular No. 50-2021 2 and indicated their respective LGUs as their place of residence should AGAIN register under this Circular, considering that they no longer wish to be inoculated in their respective LGUs of residence but instead at the Supreme Court, Manila.

This registration with the Judiciary Vaccination Registration is being undertaken to share the gathered information with the NTF and ensure that there will be sufficient supply of vaccines for all the registrants.

All those qualified and availing of the Judiciary Vaccination Program in the Supreme Court shall likewise register as soon as possible with the City of Manila Vaccination Program through manilacovid19vaccine.ph. Registrants to the Manila Vaccination Program shall indicate as their address the Supreme Court of the Philippines, Padre Faura, Ermita, Manila, Brgy. 670. They are also reminded to download and save a copy of their waiver with the QR Code which will be provided upon registration with the City of Manila Vaccination Program, as this will have to be presented at the time of inoculation.
