COVID-19 Vaccination Monitoring Report
OCA Circular No. 126-2021 mandates all Clerks of Court in the Philippines to submit a Monthly COVID-19 Vaccination Report to monitor the vaccination status of judges and court personnel. Reports are due by the 20th of each month, covering the previous month's vaccination data. The report must detail the total number of personnel, alongside counts of vaccinated and unvaccinated individuals. Once a court reports that all personnel are fully vaccinated, no further reports are necessary unless new personnel are appointed. To ensure the vaccination of all judges and court personnel nationwide against COVID-19, and for proper coordination with the National Task Force Against COVID-19, all Clerks of Court of the Office of the Clerk of Court and Branch Clerks of Court SHALL ACCOMPLISH the Monthly COVID-19 Vaccination Report in the Philippine Judiciary 365 Reports Portal on or before the 20th day of each month, for the monthly period beginning on the 16th day of the preceding month until the 15th day of the succeeding month, starting on or before 20 October 2021.

The Monthly COVID-19 Vaccination Report shall indicate the total number of personnel in each court or office (including contractual and detailed local government unit employees) and the number of vaccinated and unvaccinated personnel. Once a report has been submitted that the presiding judge and all the court personnel have been fully vaccinated, the concerned clerk of court need no longer submit subsequent reports unless new personnel are thereafter appointed in the said court or office.
