CHED MEMORANDUM ORDER NO. 065-17
| SUBJECT | : | Addendum to CHED Memorandum Order No. 51, Series of 2016 Entitled "Guidelines on Eligibility of Displaced HEI Personnel to the Grants Under the K to 12 Transition Program" |
Pursuant to Republic Act No. 7722 or the Higher Education Act of 1994, the Commission on Higher Education released CHED Memorandum Order (CMO) No. 51, series of 2016, entitled Guidelines on Eligibility of Displaced HEI Personnel to the Grants Under the K to 12 Transition Program. In line with the Commission's continued efforts to refine, nuance, and increase the responsiveness of its policies, the following addendum to CMO No. 51, s. 2016 is hereby adopted and promulgated by the Commission, for the guidance of all concerned stakeholders. ITAaHc
SECTION 1. Article V of CMO No. 51, s. 2016 is hereby amended and shall read as follows:
"Article V
Minimum Eligibility Requirements

5A. Has not been a beneficiary of the Department of Labor and Employment (DOLE) Adjustment Measures Program (AMP) or has been employed by the Department of Education (DepEd) through the (DepEd) DepEd Green Lane.

SECTION 2. A New Article XIV-A shall be inserted after Article XIV of CMO No. 51, s. 2016 to read as follows:
"Article XIV-A
Transitory Provisions and Harmonization of Grant Provisions
Grantees awarded with grants under CMO No. 51, s. 2016, prior to the issuance of this addendum, shall be governed by the following transitory provisions:
1. Grantees who have duly executed grant contracts shall be allowed to continue and finish their contracts accordingly: and
2. Provisionally approved applicants without duly executed grant contracts and prospective and ongoing applicants shall be subject to the additional eligibility requirements as described in this addendum and shall likewise comply with any additional documentary requirements as a result thereof."
SECTION 3. Appendix A of CMO No. 51, s. 2016 is hereby amended and shall now follow the format prescribed in Annex 1 of this CMO.
SECTION 4. Appendix E of CMO No. 51, s. 2016 is hereby amended and shall now follow the format prescribed in Annex 2 of this CMO.
SECTION 5. Repealing Clause. — CMO No. 51, s. 2016 and all other CHED Memorandum Orders or parts thereof which are consistent with this Act are hereby repealed, amended or modified accordingly. All other provisions of CMO No. 51, s. 2016 that are not affected by this amendment shall remain valid and in force. cSaATC
SECTION 6. Separability Clause. — If any part of this Order is declared unconstitutional or invalid, such parts or provisions thereof not so declared shall remain valid and subsisting.
SECTION 7. Effectivity Clause. — This policy shall take effect immediately after its issuance. Immediate dissemination of and strict compliance with this Order is directed.

"Appendix A: Full Implementing Arrangements for Scholarships for Graduate Studies-Local

D. Monitoring and Evaluation
Each Grantee shall be required to make the following submissions to the Commission for the duration of the scholarship:

ANNEX 2
"Appendix E: Checklist of Application Requirements
To qualify for the minimum eligibility requirements, applicants must submit the following to the Regional K to 12 Transition Program Management Unit, and the online portal when applicable.
1. Proof of Displacement (use template provided)
The Proof of Displacement to be considered valid by the Commission, must provide the following:
a. The reason or reasons why the HEI personnel's employment was terminated;
b. The inclusive dates, rank and status, teaching/workload, and corresponding number of semesters or school year when the displaced HEI personnel engaged his services with the previous HEI. This should be accomplished thoroughly by the human resource personnel, or supervisor or any other personnel of similar import within the HEI who is authorized and knowledgeable of the employee's record and performance;
c. The name and signature of the person authorized by the HEI to issue documents relating to the process of displacement; and
d. Subscribed and sworn written statement (notarized) indicating the truthfulness and accuracy of the above mentioned items.
1A. Character Reference (use template provided)
The Character Reference must be accomplished by the following:
a. An immediate supervisor, Department Chair, Dean or Head of Institution from the previous HEI; and
b. An unbiased work colleague
Both persons must have sufficient discretion and knowledge of the work ethics, intellectual and administrative abilities (demonstrated or potential), and character of the displaced HEI personnel. They must be currently employed in the above-mentioned HEI;
2. Self-certified true copy of any valid government issued ID with picture showing Proof of Filipino Citizenship (i.e., Passport, Driver's License, Voter's ID);
3. Original or Certified True Copy of Transcript of Records;
4. Original accomplished Medical Certificate issued within the last six (6) months;
5. Two (2) pieces 1 x 1 pictures taken within the last six months;
6. Original accomplished and signed Return Service Plan; cHDAIS
7. Original accomplished and signed DBP Application Form; and
8. Application document sent to the applicant after applying online, duly signed by the same.
Applications submitted with incomplete requirements shall NOT be processed."
