For uniformity in the submission of reports on Register of Notaries Public as required by Sec. 12, Rule III of A.M. No. 02-8-13-SC dated 06 July 2004 (2004 Rules on Notarial Practice) and OCA Circular No. 148-2014 dated 25 November 2014, which are to be periodically submitted on or before the fifteenth (15th) day of February and August to [email protected], you are hereby DIRECTED to use the attached template in excel form (Annex "A"). Also attached is a sample filled-out form (Annex "B") for your reference.
