Amendment to OCA Circular No. 103-2021 Template for the Report on Register of Notaries Public as Required under Sec. 12, Rule III of A.M. No. 02-8-13-SC (2004 Rules on Notarial Practice)
OCA Circular No. 132-2022 mandates all concerned Executive Judges of Multi-Sala and Single-Sala Regional Trial Courts to use a standardized template for reporting the Register of Notaries Public. This is in accordance with Section 12, Rule III of A.M. No. 02-8-13-SC (2004 Rules on Notarial Practice). Reports must be submitted quarterly, within ten days after each quarter, to a specified email address. The circular includes an Excel template and a sample filled-out form for guidance. Compliance with these directives is required.

For uniformity in the submission of data regarding the Register of Notaries Public as required by Sec. 12, Rule III of A.M. No. 02-8-13-SC dated 06 July 2004 (2004 Rules on Notarial Practice) and OCA Circular No. 111-2022 dated 17 May 2022, which are to be periodically submitted on or before ten (10) days after each quarter to [email protected], you are hereby DIRECTED to use the attached template in excel form (Annex "A"). Also attached is a sample filled-out form (Annex "B") for your reference.
