Pursuant to the Civil Service Commission Memorandum Circular No. 5, s. 2021, citing CSC Resolution No. 2100020, promulgated on 7 January 2021, effective on 24 April 2021, all judges and personnel of the first and second level courts are hereby enjoined to use the prescribed revised Application for Leave Form (Civil Service No. 6, Revised 2020) with the Instructions and Requirements, appended as Annex "A".
