COMELEC RESOLUTION NO. 9964
REVISED GUIDELINES ON THE PRINTING, REPRINTING, RELEASE, AND DISTRIBUTION OF THE VOTERS' IDENTIFICATION CARDS AND THE SUBMISSION OF REPORTS THEREON
WHEREAS, the Commission is mandated under Section 25 of Republic Act No. 8189 to adopt a design for the voter's identification card to be issued to registered voters which shall serve as a document for their identification;
WHEREAS, the Commission, in Minute Resolution 06-0044 dated 17 January 2006, resolved to approve guidelines on releasing and re-generation of voter's ID cards;
WHEREAS, pursuant to Minute Resolution No. 09-0579 dated 8 September 2009, the generation, printing and issuance of the voter's ID cards had been deferred and shall resume after the cleansing of nationwide central database through the Automated Fingerprint Identification System (AFIS);
WHEREAS, the Commission promulgated Resolution No. 9387 on 13 April 2012 entitled Guidelines on the Release, Distribution and Regeneration of the Voter's Identification Cards to provide for uniform guidelines and policy on the custody and distribution of voter's ID cards at the Office of the Election Officer level;
WHEREAS, there is a need to revise the above resolution in order to provide for reportorial requirements for purposes of maintaining an accurate inventory of voter's ID cards and to respond to issues presented on this subject;
WHEREAS, pursuant to Minute Resolution No. 15-0202 dated 10 March 2015, the Commission approved with modification the Guidelines on the Printing, Reprinting, Release, and Distribution of Voter's Identifications Cards;
NOW THEREFORE, pursuant to the powers vested in it by the Constitution, the Omnibus Election Code, Republic Act 8189 and other related election laws, the Commission RESOLVED, as it hereby RESOLVES to adopt the following REVISED GUIDELINES ON THE PRINTING, REPRINTING, RELEASE, AND DISTRIBUTION OF THE VOTERS' IDENTIFICATION CARDS AND THE SUBMISSION OF REPORTS THEREON, as follows:
I. Procedure Before Printing of Voter's ID Cards
1. Within the period provided in pertinent resolutions of the Commission on Elections, which shall herein after referred to as the "Commission", an applicant shall personally file an application for registration (including application for transfer of registration record, re-activation, change of name/status, correction of entries) with the Office of Election Officer (OEO).
2. All applications filed shall be heard by the Election and Registration Boards (ERBs) of every district/city/municipality, on a quarterly basis, in accordance with the schedule set in the pertinent resolutions.
3. Within a maximum period of five (5) days after every ERB hearing, the database (a.k.a. SNPDAT) of all approved applications shall be forwarded to the Commission's Main Office in Manila, through the Information and Technology Department (ITD).
4. The SNPDAT shall then be uploaded by ITD to the data server for Automated Fingerprint Identification System (AFIS) processing where each record shall be matched with all other records nationwide to determine double/multiple registrations.
All records found to match other records in the data server shall be treated as "hits" and thereafter printed, consolidated and endorsed by ITD to the OEO for deletion in the local database.
Records identified as "hits" and those found to have incomplete/unreadable/poor quality of biometrics data shall not be printed their corresponding voter's ID cards.
5. Registration records without problems/irregularities shall be printed their voter's ID cards and thereafter proceed to manual receiving, sorting, verification and packing (RSVP) by ITD.

II. Procedure After Printing of Voter's ID Cards
A. Procedure on the release of voter's ID cards
A.1. From the Commission's Main Office in Manila, through IRD, to Offices of the Provincial Election Supervisors (OPESs) in case of municipalities or to OEOs in case of cities
1. The ITD shall, after printing the voter's ID cards, ensure that each properly packed bundle contains the following:
a. Packing List;
b. CD or hardcopy containing the list of printed Voter's Registration Record (VRR) and voter's ID cards;
c. If applicable, CDs or hard copy containing the list of unprinted VRR and voter's ID cards;
d. Actual VRR/voter's ID cards.
2. The ITD shall then transmit the bundles of the voter's ID cards to IRD, which shall ensure that the bundles shall be shipped using the fastest means available to OPESs/OEOs, along with the necessary shipping documentation.
In case of OEOs in the National Capital Region (NCR), the EOs shall directly pick up the bundles of voter's ID cards from ITD after being informed of the availability thereof. The EOs shall then perform the procedures laid down in paragraphs A.2 to A.3 below.
3. IRD processing time — from receipt of bundles to dispatch — shall take approximately five (5) working days. Thereafter, there shall be a Dispatch Plus One Day (D+1) count for shipment processing by either Private Courier or PhilPost. Expected date of receipt by OPES/OEO, after necessary processing and depending on location, shall take approximately three to fifteen (3-15) working days. Total period shall be a maximum of twenty one (21) working days from IRD to OPES/OEO. In the event of any service interruption or delay, all affected parties shall be notified by the IRD.
In all, from registration to RSVP, as mentioned in Part I above, up to the shipping of voter's ID cards to the OPESs/OEOs, a total of two hundred six (206) days or 6 3/4 months for five hundred thousand (500,000) records and three hundred fifty one (351) days or 11 3/4 months for a maximum of one million eight hundred thousand (1,800,000) records shall be necessary before voter's ID cards shall become available for distribution to voters.
A.2. From OPESs to OEOs
1. Within two (2) working days from receipt of the bundles of voter's ID cards, the OPESs shall conduct inventories of the bundles received to verify if the total number thereof is the same as that indicated in the IRD's Tracking and Billing Form (TBF). Any discrepancy in the total number of the bundles of voter's ID cards shall be immediately reported, through the fastest means available, to IRD. Until such discrepancy is resolved, the OPESs shall hold the delivery of the bundles of voter's ID cards to the OEOs.
2. If everything is in order, the OPESs shall, within three (3) working days from completion of inventories, cause the deliveries of the bundles to the OEOs. For this purpose, the Commission shall provide OPESs with sufficient allocations to specifically shoulder the expenses for such deliveries subject to standard auditing rules and regulations.
3. The IRD shall furnish the ITD with the List of Tracking Numbers issued within five (5) working days from dispatch. The IRD shall likewise submit to ITD a report on "Receiving Information" based on proof of delivery at least once a month.
A.3. From OEOs to voters
1. Procedure before distribution of Voter's ID Cards to voters
Within three (3) working days from receipt of the bundles of voter's ID cards, the EOs shall:
i. Check the packing lists containing the names of voters by precinct per barangay in the attached CDs/hardcopies as against the actual voter's ID cards received. Any discrepancy shall be reported through the fastest means available to ITD. Until such time that the discrepancy is resolved, the EOs shall have the right to withhold the Acknowledgement Receipts (ARs) but other activities enumerated below shall be performed except the posting of the lists and releasing of voter's ID cards to voters.
The phrase "any discrepancy" shall mean to include not only inconsistency in the total number of voter's ID cards received but also to the details contained therein such as typographical/substantial errors in the voters' photos, names, precincts assignments, etc.
ii. If there is no discrepancy, sign the AR then send a copy thereof to ITD, either through fax or e-mail it to itd.comelec.gov.ph before sending the AR's original copy to the same Department.
iii. After sending the AR to ITD, tag the newly received voter's ID cards of active voters using the "ID On-Hand" application in the Voter's Registration System (VRS).
iv. While tagging the newly received voter's ID cards, segregate the voter's ID cards of the following voters whose registration records were:
a. Deactivated;
b. Cancelled by reason of death;
c. Transferred to another precinct within the same barangay/district/city/municipality or corrected as regards the names/status or any other entries therein;
d. Transferred to another district/city/municipality;
e. Corrected as regards the name, status or any other entries therein.
All voters' ID cards segregated due to any of the enumerated grounds above shall be withheld by the EOs subject to the policy on disposal of public records, except those voter's ID cards withheld due to deactivation. Voter's ID cards withheld due to deactivation shall be segregated but kept in the custody of the OEOs in the event of the approval of reactivation without a change in the details of the voter.
v. After tagging, alphabetically consolidate the newly received voter's ID cards of active voters with those still in the possession of the OEO by precinct per barangay.
vi. Generate from the VRS, a new list containing the names of voters whose voter's ID cards, regardless of whether newly received or not, are in the possession of the OEO. Such list shall indicate that the voter's ID cards are available for release at the OEOs. The procedure on how to claim the voter's ID cards provided in Part II, A.3, No. 2, from letter a to e, shall be integrated in the list.
vii. Post the generated list in the bulletin boards of the OEOs and of the barangays, if any, or in the barangay halls and in three (3) other conspicuous places in the barangays.
viii. Notify the voters, through registered mail, that their voter's ID cards are already available for release in the OEOs.
On top of the regular office supplies requirement, materials, i.e., coupon bonds, printers, ribbons for printers, filing cabinets, and other provisions for mailing of individual notices and posting of the lists of voters whose voter's ID cards are available for release at the OEOs, which shall be needed to carry-out the instructions enumerated above, shall be provided by the Commission to the OEOs on a quarterly basis or as the need arises upon request by the OEO through the OPES.
2. Procedure during and after distribution of Voter's ID Cards to voters
a. A voter may personally appear at the OEO on any working day from 8:00 AM to 5:00 PM to claim his/her voter's ID cards.
b. Only voters whose registration records are active may be issued their voter's ID cards.
"Active voters" refer to those registered voters whose names are included in the active list of voters or whose VRRs are included in the Precinct Book of Voters. Also, those whose names are not cancelled, deleted or deactivated. It shall also include those whose registration records were previously deactivated but whose subsequent applications for reactivation were approved. In that case, unless the applications for reactivation was accompanied either by change in precinct assignments, names, status or correction in any other entries in the registration records, the original voter's ID cards shall be issued to the voters.
c. A voter may authorize in writing any person of legal age, to claim his/her voter's ID cards provided that the representative shall be able to present any valid ID or, in the absence thereof, any valid ID of the voter being represented with his/her signature. In case more than one (1) voter's ID cards shall be claimed, the representative shall present any of his/her valid ID, or in the absence thereof, any valid ID of all the voters being represented with their signatures.
d. To ensure proper disposition of voter's ID cards, the representative shall fill-up a Sworn Statement with Undertaking (Annex "A") which shall contain a statement that he/she shall deliver the voter's ID card/s to the voter-owner/s thereof.
e. Before releasing the voter's ID card/s, the EO shall require the person claiming the same to sign in the log book provided for the purpose. In case of a representative, in addition to his/her signature, he/she shall indicate thereon that the voter's ID card/s was/were claimed on behalf of the voter-owner/s thereof. All valid IDs presented, belonging either to the representative or the voter-owner/s, shall likewise be indicated in the log book.
f. In all instances, when a voter's ID card is released, either to the voter or his/her representative, the EO shall, before the close of working hours on the day the voter's ID card is claimed, make the necessary update by tagging the registration record of the voter using the VRS to show that the voter's ID card was already claimed.
In case more than one voter's ID cards were printed for a voter due to updating of his/her record in the database submitted by the EO to ITD, the EO shall determine which voter's ID card is the latest and issue the same to the voter free of charge. The old voter's ID card, if already issued to the voter, shall be surrendered to the EO before the new voter's ID card shall be released. The old voter's ID card shall be disposed subject to the policy on disposal of public records.
Verification of the status of printing voter's ID cards may be made through the Commission Website's Precinct Finder.
3. Venue of distribution of Voters' ID Cards
a. Voters' ID cards shall be distributed at the OEOs.
No voter or representative shall be allowed to claim a voter's ID card at the Commission's Main Office in Manila.
b. Distribution may also be done at the barangay halls or in any other public places/buildings in the barangays. For this purpose, the EOs shall coordinate with barangay officials for the schedule of distribution of voter's ID cards. The procedures laid down in Letter A.3, No. 2, c, d, e and f above shall be observed in the distribution of voter's ID cards, which are hereunder re-stated:
i. A voter may authorize in writing any person of legal age, to claim his/her voter's ID card, provided that the representative shall be able to present any valid ID or, in the absence thereof, any valid ID of the voter being represented with his/her signature. In case more than one voter's ID card shall be claimed, the representative shall present any valid ID, or in the absence thereof, any available valid ID of the voters bearing their signatures. (A.3., No. 2, c.)
ii. To ensure proper disposition of voter's ID cards, the representative shall fill-up a Sworn Statement with Undertaking (Annex "A") which shall contain a statement that he/she shall deliver the voter's ID card/s to the voter-owner/s thereof.
iii. Before releasing the voter's ID card/s, the EO shall require the person claiming the same to sign in the log book provided for the purpose. In case of a representative, in addition to his/her signature, he/she shall indicate thereon that the voter's ID card/s was/were claimed on behalf of the voter-owner/s thereof. All valid IDs presented, belonging either to the representative or the voter-owner/s, shall likewise be indicated in the log book. (A.3., No. 2, e.)
iv. Upon returning to the OEO, the EO shall, before the close of working hours on the day of the distribution of voter's ID cards, if possible, or on the next working day immediately after the distribution of voter's ID cards outside the OEO, make the necessary update, using the VRS, by tagging the registration records of voters whose voter's ID cards were released, to show that said Voter's ID cards were already claimed. (A.3. No. 2. f)
Distribution may also be done during the conduct of satellite registrations provided that the procedures stated above shall be followed.
All distribution of voter's ID cards made outside the OEO shall be done personally by the EO/Election Assistant (EA) or upon his/her direct supervision and shall be at no cost to the Commission.
If the scheduled distribution of voter's ID cards in barangay halls or in any other public places/buildings, other than the OEOs, shall be more than one (1) day, the unclaimed voter's ID cards after the first day and so on shall not be left behind but returned to the OEOs.
III. Printing and Reprinting of Voter's ID Cards
A. Grounds for Printing of Voter's ID Cards
1. Newly approved and validated registration records;
2. Transfer of residence within the same barangay/district/city/municipality which involves change in precinct assignment and/or accompanied by an application for change of name/status or correction of entries;
3. Transfer of residence to another district/city/municipality;
4. Transfer with reactivation;
5. Correction of entries;
6. Change of status;
7. Change of precinct assignment of voters due to alteration of boundaries/re-mapping of local government units (LGUs) which shall affect the territorial boundaries of mother/original precinct/s;
8. Incomplete/unreadable/poor capturing/recording of biometrics data.
B. Ground for Re-printing of Voter's ID Cards
1. Lost, torn, damaged or defaced voter's ID cards
C. General Procedures
1. For Letter A, Nos. 2 to 6, the voters shall accomplish in the OEO a Request for Printing of New Voter's ID (Annex "B") and surrender their old voter's ID cards, if any. The fact of turning over the old voter's ID cards shall be indicated in the request. If the voters cannot present the voter's ID cards, a Sworn Statement of Loss (Annex "C") shall be submitted instead. Copies of such statements shall be attached to Annex "B" and kept for record purposes by the EO.
2. For Letter A, No. 7, the EO shall fill-up a Request for Printing/Re-printing of Voter's IDs by Election Officers (Annex "D") and forward the same to EBAD which shall come up with a recommendation for the printing/re-printing of new voter's ID cards of voters who shall be affected by the boundary alteration/re-mapping by LGUs to the Commission En Banc. The database showing changes in precinct assignments shall be forwarded to ITD by EBAD.
Upon receipt by the EO of the new voter's ID cards and before they shall be released to the voters, the EO shall require the voters to surrender their old voter's ID cards, if already released to them prior to the boundary alteration/re-mapping. The old Voter's ID cards shall be disposed subject to the policy on disposal of public records.
If the voters can no longer present their old voter's ID cards, a Sworn Statement of Loss (Annex "C") shall be submitted instead which shall be kept for record purposes by the EO.
3. For Letter A, No. 8, if voter's ID cards cannot be printed due to incomplete/unreadable/poor quality of biometrics data, such as fingerprint impressions, dim, blurred or improperly cropped photographs, and those that are similarly flawed, the following procedures shall be observed:
i. The ITD shall prepare the list of voters whose voter's ID cards cannot be printed due to any of the foregoing reasons and transmit said list to the OEO.
ii. Upon receipt of the list, the EO, through registered mail, shall notify the voters thereof and instruct them to appear in the OEO for the recapturing of their biometrics data.
iii. The EO shall submit to ITD, on a quarterly basis, the SNPDAT and the list of voters who underwent recapturing of biometrics data with a request for printing of voter's ID cards.
The printing of voter's ID cards due to any of the grounds under Paragraph A shall be free of charge.
4. For Letter B No. 1, in case of lost, torn, damaged or defaced voter's ID cards, the following rules shall be observed:
i. If voter's ID cards were lost, torn, damaged or defaced, while in the possession of the voters, they shall fill-up in the OEO Annex "B". Attached the torn, damaged, or defaced voter's ID cards, if available, or, if not, a Sworn Statement of Loss (Annex "C") and pay one hundred pesos (P100.00) each. The total amount paid by the voters shall shoulder the costs of re-printing and delivery of the new voter's ID cards to the OEO. Copies of the statements shall be attached to Annex "B" while the original copies thereof shall be kept for record purposes by the EO.
The EO shall issue an Official Receipt (OR) for every amount received. The original copy of the OR shall be given to the voters while a photocopy thereof shall be attached to Annex "B".
ii. If voter's ID cards were lost, torn, damaged or defaced while in the OEO, without the EO's fault, such as due to natural calamities, accidents or any other similar instances beyond his/her control and after exercising due diligence to avoid or minimize the loss, defacement or destruction of voter's ID cards, the EO shall notify the ITD thereof in writing together with the list of voters whose voter's ID cards were lost, torn, damaged or defaced for re-printing. For such purpose, the EO shall accomplish Annex "D". A brief narrative stating the proximate cause/s of such loss, damage or defacement of voter's ID cards shall likewise be appended in the request.
iii. If voter's ID cards were lost, torn, damaged or defaced while in the OEO due to the EO's fault, negligence or non-compliance with any of the provisions herein contained, he/she shall notify the ITD thereof in writing together with the list of voters whose voters' ID cards were lost, torn, damaged or defaced for re-printing. For such purpose the EO shall likewise accomplish Annex "D". In addition, the EO shall pay One Hundred Pesos (P100.00) for every voter's ID card lost, torn, damaged or defaced. The total amount paid shall shoulder the costs of re-printing and delivery of the new voter's ID cards to the OEO. The foregoing are without prejudice to the filing of an administrative case against the EO.
All registration records for printing and re-printing shall undergo AFIS cleansing.
Request for printing/re-printing shall be consolidated and forwarded by the EOs to ITD on a regular basis or as the need arises. For this purpose, only soft copies of Annexes "B" and "D" shall be submitted to ITD.
IV. Safekeeping Duties
The following shall be kept in the OEOs and classified as voter's ID records:
1. All unclaimed voter's ID cards of active voters;
2. All voter's ID cards withheld by the EOs for justifiable grounds;
3. Completely filled-up Annexes;
4. Photocopies of ORs;
5. Turned-over voter's ID cards subject of request for printing/re-printing; and
6. Log book showing the name of voters whose voter's ID cards were already claimed.
EOs due for retirement, promotion or transfer to another district/city/municipality shall ensure proper turn-over of the foregoing to their successor. Failure to do so shall constitute a violation of these guidelines.
V. Treatment of Voter's ID Cards Printed and Distributed to OEOs from 2004-2009
Voter's ID cards released from 2004 to 2009 which can no longer be accounted for by the OEOs shall be re-printed upon written request of the voters at no cost to them.
Inquiries on status of voter's ID cards made to ITD through e-mails or phone calls shall be validated first. If found to be in the possession of the EOs as shown in the ID On-Hand reports, the voters shall be advised to proceed and claim the same to the OEOs.
If not found in the ID On-Hand reports, such fact shall be reported by ITD to the EOs for further verification. If the verification turned negative, the EOs shall fill-up Annex "D" and submit the same to ITD. The EOs shall inform the voters of the actions taken regarding their queries.
VI. Voter's Certification as Temporary Voter's ID Card
The EO shall issue voter's certifications which shall serve as temporary voter's ID cards upon the request of registrants whose applications for registration have been approved by the ERB and payment of the amount of One Hundred Pesos (P100.00). The EO shall issue an OR for every amount received.
The temporary voter's ID card shall be valid for a period one (1) year from the date of its issuance and shall be subject to the results of the AFIS and/or relevant proceedings assailing the registration of a voter.
The EO shall maintain an accurate inventory of all temporary voter's ID card issued in their respective Offices and shall ensure proper turnover of inventory in the event of employee re-shuffling. The temporary voter's ID card shall be surrendered prior to the release of the permanent voter's ID card. If the voter cannot present the temporary voter's ID card, a Sworn Statement of Loss (Annex "C") shall be submitted instead.
A proposed template of the temporary voter's ID card is hereto attached as Annex "E". The ITD shall coordinate with EBAD for purposes of fixing the details necessary to be stated in the temporary voter's ID card. Thereafter, the ITD shall create a program/software/patch for the use of the OEO in printing the temporary voter's ID card.
VII. Quarterly Reporting on Voter's ID Cards Inventory
Data on the total number of voter's ID cards tagged, withheld, released as well as those requested for printing/re-printing, shall be submitted on a quarterly basis in soft copies to the Election and Barangay Affairs Department (EBAD) through [email protected] or [email protected] using the format of Quarterly Status Report on Voter's IDs Releases (Annex "F"). Such data shall be simultaneously submitted with the Quarterly Progress Reports (QPRs) to the same Department. Copies of Annex "D" shall be furnished to the Education and Information Department (EID) through [email protected], OPESs and Offices of the Regional Election Directors (OREDs).
Reports on voter's ID cards "On-Hand" and "Released" shall be submitted in soft copy to ITD on a quarterly basis.
VIII. Prohibited Acts and Penalties
The EO or any other person acting directly/indirectly under his/her instruction/supervision, who shall commit any act/s in violation of any of the foregoing provision/s, intentionally, or through inexcusable/gross negligence, or with fraud or intimidation, shall be held liable for administrative offenses and withholding/forfeiture of all benefits (including retirement benefits, if warranted) and other monetary emoluments due him/her, without prejudice to the imposition of other applicable penalties under the Civil Service rules and regulations.
The printing, reprinting, release and distribution of fake or spurious voter's ID cards by any person, group of persons or entity shall be penalized for falsification of public document and meted out the corresponding penalty under the Revised Penal Code (RPC) and any applicable special penal laws.
Let the relevant Departments and Offices implement this Resolution.

I fully understand that I will be liable for perjury, theft, estafa or any other criminal offense, as the case may be, should it be established that I am not the duly authorized representative to receive the said temporary voter's ID card/voter's ID card, or that I failed to deliver the same to its rightful owner.
