Vargas Room 5 Mezzanine Floor Marimas Bldg., 2228 Rizal Ave., Olongapo City Madam : This has reference to your letter dated July 3, 1981 requesting informations regarding the Olongapo Civic Action Group (OCAG). llcd In connection therewith, please be informed that subject corporation was incorporated on January 25, 1967. However, to date, it has not filed its by-laws with this Commission. As per its Articles of Incorporation, the incorporating directors are eleven (11) and their names and residences are as follows: NAMES RESIDENCES 1. Anastacio R. Bernal Olongapo City 2. Amelia J. Dominador Farin Olongapo City We can not furnish you with the informations requested in letters e, d, and f of our letter since the subject corporation has not been submitting its annual reportorial requirements. The last communication was dated January 4, 1967. A verification of the records of this Commission does not disclose that Boys Town and Girls Home is a project of Olongapo Civic Action Group.
