December 21, 2011 SEC NOTICE TO : All Planholders of Metropolitan Life Plan Company, Inc. Notice is hereby given that on 25 February 2010 the Commission resolved to continue with the implementation of the liquidation program/buy-back scheme of Metropolitan Life Plan Company, Inc. Under the said scheme, all fully paid and outstanding life plans as of June 30, 2008 shall be paid at 50% of the gross pre-need price, and all life plans matured and surrendered as of June 30, 2008 shall be paid based on the Memorial Service Value plus the Cash Assistance pursuant to the life plan contract. In view of the foregoing, please submit the following documents to the Non-Traditional Securities and Instruments Department, at 10th Floor, SEC Building, EDSA, Greenhills, Mandaluyong City, or to our nearest extension office: 1. Original copy of plan contract, or if lost, an affidavit of loss of said document; 2. Original copy of certificate of full payment of life plan, or if lost, an affidavit of loss of said document; 3. Photocopy of death certificate of planholder, if applicable; 4. Photocopy of marriage contract of the beneficiary, if applicable; 5. Photocopy of birth certificate of the beneficiary, if applicable; 6. Authorization letter of authorized representative, if applicable; and 7. Original and photocopy of two (2) valid identification cards of the planholder/beneficiary and authorized representative, if applicable.
