April 30, 2020 SEC NOTICE TO : Capital Market Institutions, Participants and Employees RE : Guidelines for the Applications for RapidPass QR Codes for Those Who Have Been Issued IATF IDs In line with its task to fast track movement and access of frontliners at designated checkpoints during the Enhanced Community Quarantine, the Department of Information and Communications Technology ("DICT") in partnership with DEVCON Community of Technology Experts ("DCTx") launched the RapidPass System. Under the RapidPass System, individuals who are exempted from the strict work from home requirement under Resolution No. 13 of the Inter-Agency Task Force ("IATF") shall be given a unique digital pass with QR Code which checkpoint personnel will scan for easier identification. 1 LIST OF REQUIREMENTS 1. Completely accomplished Bulk Registration Form 2. Signed Attestation Form The forms can be accessed here: https://dict.gov.ph/rapidpass/ . In accomplishing the forms, please be mindful of the following: a) Make sure to complete the * Required Fields in the forms. b) Do not delete any columns as the sheet is strictly matched in the system. c) The attestation form must be in PDF format while the registration forms must be in CSV formats. To ensure that the CSV format is in accordance with the system of RapidPass, save/convert the Registration Form using Google Sheets. GUIDELINES 1. Only holders of IATF IDs shall be eligible to apply for a RapidPass. Applications shall be sent to the Commission's designated e-mail: [emailprotected]. 2. Kindly note that each rider of a vehicle should be registered individually. One unique QR Code is for one person only. 3. The designated representatives of the capital market participants must use their official emails in submitting the forms. 4. All generated QR codes will be sent to both the emails and mobile numbers registered. Thus, capital market participants shall ensure that all data in the registration forms are correct and valid. 5. Duplicate submissions from the same organization or company shall be denied. FREQUENTLY ASKED QUESTIONS For a better understanding of the RapidPass requirements and procedures, please read the attached FAQs. For your guidance. ATTACHMENTS Frequently Asked Questions 1. I work with a Capital Market participant. Can I apply for a RapidPass? If you have been issued an IATF ID by the SEC, you are qualified to apply for a RapidPass. 2. Can my officemates also apply? Yes, if they have been issued IATF IDs. 3. How do we apply? Individually or as a group? It is more convenient and faster if you apply as a group, that is, through bulk registration . 4. How do we apply using this bulk registration? Your office has to designate a person (preferably someone doing HR and/or admin work) to accomplish the bulk registration form. The individual applicants have to provide the required information. To access this form, please refer to the Notice. This is also available in the DICT website under RapidPass. 5. What information do I have to provide? Each individual applicant is required to provide fifteen (15) data points which are the following: Pass Type Always input "Individual." Nature of Work Always input "CM" to indicate Capital Market. First Name Input info as required. Middle Name Not a mandatory field. Last Name Input info as required. Suffix Select info based on dropdown. Company Institution Name of the company Identity Document Type To facilitate our review before endorsing the application to DICT, we advise you to instead indicate "IATF." Identity Document No. Indicate the number appearing in the IATF ID used. Plate No. No need to fill up. DICT has stopped issuing vehicle RapidPass ID. Mobile Indicate the mobile phone number of the individual applicant based on format. Email Indicate the email address of the individual applicant based on format. Origin Name No need to fill up. Home Street of Personnel Not a mandatory field. Home City of Personnel Indicate the city of the individual applicant where s/he resides. Home Province of Personnel Indicate the province of the individual applicant where s/he resides. Destination Name No need to fill up. Destination Street Not a mandatory field. Destination City The SEC requires you to fill this up as part of its approval and endorsement process. What do we do next after accomplishing the bulk registration form? Check that all the required fields are correctly filled up to avoid delay. Do not delete any of the columns if it is not a mandatory field. Submit the documents to this email address [emailprotected] together with the Attestation Form. 7. What is an Attestation Form? It is a document that certifies that the information in the Bulk Registration Form pertaining to the individual applicants are true and correct. It also certifies other statements contained in the templated Attestation Form. To get the form, please access the RapidPass section of the DICT website. 8. Who is required to execute the Attestation Form? Any responsible officer of the company. It is advised that the officer has the proper authority or has been granted such authority. 9. Do I need to register my vehicle? No, vehicle registration is not necessary. All the riders of a vehicle, however, must have their own QR code. 10. Is it necessary for the applicant to have his/her own vehicle? No, a person can apply regardless if s/he has a vehicle. 11. I do not have my own e-mail address. Can I indicate the email address of the company's Human Resources Department? Yes, the system accepts applications of different people with the same email address. Note that all the QR codes of the persons using that email address will be sent to the same address. 12. What should I indicate in the column for Nature of Work in the Registration Form? For applicants under the jurisdiction of the Commission, the Nature of Work should always be "CM." 13. What should I indicate in the column for Identity Document Type in the Registration Form? Applicants under the jurisdiction of the Commission are strongly advised to use their IATF IDs as the identity document. Hence, please indicate "IATF." 14. How do I know that my registration is approved? You will receive an email/SMS with the approved QR Code. 15. Can other people use my digital pass with QR code? No. Your unique QR code is tied to your user information, and valid ID. The QR code can only be used by the approved person or personnel registered in the system. 16. Do I need another ID if I have a RapidPass.ph? Yes, checkpoints may still ask for your valid ID along with your digital pass with QR code. We recommend having your valid company ID and IATF ID with you while travelling. 17. Can I still pass through checkpoints without the RapidPass.ph? Yes, you may still go through checkpoints without a digital pass. The aim of RapidPass.ph is to make the checkpoint process easier for both front-liners and the checkpoint personnel. Proper documentation and valid IDs will still be acknowledged at designated checkpoints. 18. How long is my QR code valid? There is varying validity of the QR code, depending on the approval given. Please check your approval email for the validity of the QR code. 19. If the ECQ is extended beyond the end of the validity period of the QR code, do I need to re-apply for a new one? There is no need to re-apply. The system will automatically generate a new QR code and send it to the approved applicants. 20. My work as a messenger entails me to go to different destinations. Can I apply for a QR code for multiple destinations? Yes, you may indicate "Multi" in the Destination City and Destination Province columns in case of multiple destinations. Otherwise, indicate the specific city or province. Note that SEC will still review the necessity of multiple destinations and has the discretion not to allow an application indicating "multi" if not justified. In this case, you are required to submit a separate justification via email sent to [emailprotected]. 21. I am not a Capital Market participant and I do not know to which agency should I submit my application for a QR code. What should I do?
