May 22, 2003 SEC MEMORANDUM CIRCULAR NO. 05-A-03 ADDENDUM TO SEC MEMORANDUM CIRCULAR NO. 5, SERIES OF 2003, RE: IMPLEMENTATION OF THE SELF-RATING SYSTEM ON CORPORATE GOVERNANCE In addition to the companies enumerated in said Circular, the following entities are likewise required to submit their duly accomplished Corporate Governance Self-Rating Forms (CG-SRF) not later than July 31, 2003, to wit: 1. Government Securities Eligible Dealers (GSEDs); 2. Universal Banks Registered as Underwriters of Securities; 3. Investment Company Advisers; 4. Transfer Agents; 5. Clearing Agency and Clearing Agency acting as Depository; and 6. Issuers of Registered Time Shares, Proprietary and Non-Proprietary Membership Certificates.
