FOR: LIFTING OF ORDER OF REVOCATION O R D E R Petitioner, Winaca Foundation, Inc., is hereby directed to submit to the Commission the following: a) certificate of registration and articles of incorporation of the foundation; b) supplemental petition alleging valid and justifiable grounds why the order of revocation should be lifted; c) the accreditation of the external auditor by the Board of Accountancy or BOA accreditation; d) reconstructed copies of the pages with entries of the Membership book certified by the Corporate Secretary and 2008 business permit (if already available) within ten (10) days from date hereof.
