May 30, 1979 REVENUE MEMORANDUM CIRCULAR NO. 38-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One (1) Set of Revenue Tax Receipt (CB Form No. CB RCO 02.02) bearing Serial No. 1353766 Notice is hereby given of the loss of one (1) set of Revenue Tax Receipt, to wit: Serial No. Quantity 1353766 One (1) Set The above Revenue Tax Receipt was reported to the Acting Revenue Regional Director, Revenue Region No. 8, Tacloban City, thru the Officer-In-Charge, Revenue District No. 64, as missing or lost sometime on March 6, 1979 by MARIANO C. PEDRAZA, JR., Revenue Collector I, Office of the Collection Agent, Catbalogan, Samar.
