November 21, 1994 REVENUE MEMORANDUM ORDER NO. 68-94 SUBJECT : Revising the Use, Preparation and Distribution of the Monthly Summary of Tax Returns Filed (BIR Form 1770) TO : Regional Directors, Revenue District Officers, Chief, Statistics Division and Others Concerned To further improve the reporting and monitoring of tax returns filed in the district offices, the prescribed format of the Monthly Summary of Tax Returns Filed under Revenue Memorandum Order No. 16-92 , is hereby revised and numbered as BIR Form No. 1770 (See Annex A) . The preparation and distribution procedures are also revised such that each Revenue District Office shall now prepare BIR Form No. 1770 in duplicate and submit the original copy to the Regional Director not later than the 15th day of each month . The duplicate copy shall be retained as file. The Regional Director shall then prepare in duplicate a Consolidated Monthly Summary of Tax Returns Filed (See Annex B) and submit original copy of said summary to the Chief, Statistics Division not later than the 20th of the following month . The duplicate copy shall be retained as file. This shall apply for tax returns received starting January, 1995 . RENE G.
