September 8, 1967 REVENUE MEMORANDUM ORDER NO. 63-67 SUBJECT : Amending the Present Procedure of Requisitioning Official Receipts (BIR Form No. 25.24) TO : All Regional Directors, Revenue District Officers, Revenue Collectors and Others Concerned To facilitate the requisitioning of official receipts (BIR Form No. 25.24) by revenue collectors the country-over, particularly at times of urgent need for them, the present system of requisitioning these accountable forms is hereby reset, to wit: cdt 1. Chiefs of Administrative Branches of all revenue regions, except revenue regions 6, 7, 8, are hereby designated custodians of accountable forms, and every such officer shall be bonded, or bondable, and his fidelity insured, in accordance with the provision of the Public Bonding Law. Bond application forms for the purpose shall be sent to them by the General Services Division. 2. Initially the Chief of the Property Division will supply all bonded chiefs of administrative branches with pads official receipts sufficient for three-months use of revenue collectors in each region. Thereafter, the Chief of the Property Division shall send them official receipts as they (Chiefs of Administrative Branches) will requisition from time to time to replenish their stock. For all official receipts received by them, chiefs of administrative branches shall render a report of accountability for accountable forms (General Form No. 16(a) to the Auditor General every month and upon transfer or reassignment. 3. Henceforth, revenue collectors shall send their requisition for official receipts (using the usual General Form No. 45-A), thru their revenue district officers, to the Revenue Regional Director, Attention: The Chief, Administrative Branch.
