October 28, 1970 REVENUE MEMORANDUM ORDER NO. 57-70 SUBJECT : Reorganizing the BIR Forms Committee and Its Functions In order to implement fully the provisions of the Memorandum of the undersigned dated February 3, 1970, requiring all units to submit to the Forms Committee for approval before printing, all forms/report formats/ form letters devised by them for their use, and to involve all departments of the Bureau in the responsibility of creating and revising all documents used by operating units within their respective jurisdiction, the Forms Committee is hereby reorganized with the following membership: Revenue Operations Head (Mgt. Plg.) Chairman All Assistant Department Heads Members Chief, Org. & Methods Division Secretary The primary responsibilities of this Committee shall be: 1. To formulate and recommend policies and procedures and to devise a system for establishing and administering a Forms Control Program in the Bureau; 2. To revise, devise, or create forms/report formats/ form letters which shall be the basis for evaluating performance and the preparation of statistics and other management reports; 3. To review and approve proposed or revised forms prepared by any unit in the Bureau; 4. To assign control numbers to all forms approved for printing; 5. To cause the printing of all approved forms; and 6. To prepare and submit reports on the progress of its work. To facilitate the work of the Committee, each member shall create a sub-committee within his department to be responsible for the preliminary review of proposed forms or formats of reports to be used by any unit within his area of work before they are presented to the mother committee for final approval. This includes forms proposed by units in the regional or district offices which has a direct bearing on their functions. The Organization & Methods Division shall be the technical arm of the Committee. It shall be responsible for making detailed studies for the Committee on any proposals received by the latter. It shall be responsible for keeping records of all the activities of the Committee and shall have custody of all forms or papers submitted for review. The Committee shall render regular quarterly progress report to the Assistant to the Commissioner who shall be the overall coordinator of this program. All orders, circulars or memorandum which are inconsistent with the provisions of this Order are hereby revoked or modified accordingly. This Order takes effect immediately. MISAEL P.
