October 14, 1974 REVENUE MEMORANDUM ORDER NO. 50-74 SUBJECT : Loss of Accountable Forms Subject to Disciplinary Action TO : All Accountable Officers and Others Concerned It has been noted that the incidence of losses of accountable forms like revenue official receipts, documentary and science stamps, have become prevalent. These losses, subsequent investigation generally disclosed, have been due to carelessness and lack of precautionary measures usually exercised by custodians of accountable forms. It is probable that the Government may have incurred huge losses in revenue as a result thereof. All accountable officers and the supervisors of accountable officers concerned are hereby warned that henceforth any reported loss of accountable forms shall be rigidly investigated and the accountable officer and/or supervisor concerned shall be suspended immediately and charged administratively as the circumstance surrounding such loss may warrant. Likewise, in case the accountable form lost have money value on the face thereof, such accountable officer jointly with the supervisor concerned shall be responsible for the money value of the accountable forms lost.
