May 12, 1966 REVENUE MEMORANDUM ORDER NO. 39-66 SUBJECT : Requisitions for Accountable Forms and Supplies TO : All Collection Agents, Cash Clerks and Others Concerned In order to have a more effective control of accountable forms and supplies and to prevent the overstock thereof in the hands of Collection Agents and/or Cash Clerks no requisition (Gen. Form No. 45(A) Requisition and Issue Voucher) for accountable forms and supplies will hereafter be entertained unless the same is accompanied with an inventory of the stock thereof on hand. With respect to Official Receipt (BIR Form No. 25.24), Residence Tax Certificates (BIR Form Nos. 9.01, 9.02, 9.03(C), and 9.03(C-1), the inventory shall state the kind and quantity of the unused stock of such forms on hand, together with their corresponding serial numbers. As regards documentary and strip stamps, the inventory shall likewise state the quantity, denomination, and value of each kind of stamps that was already disposed of and sold to the public and the remaining balance thereof on hand. All Collection Agents and/or Cash Clerks who have an over supply of these accountable forms are hereby instructed to return the same immediately to the Chief, Cash and Disbursement Division of this Bureau, retaining only enough quantity thereof to last for about six (6) months. This Order takes effect immediately and strict compliance therewith is hereby enjoined. (SGD.) MISAEL P.
