Requisition of Letters of Authority and Letters of Confirmation

July 16, 1975 REVENUE MEMORANDUM ORDER NO. 36-75 In accordance with the decision arrived at in the last conference of regional directors, Letters of Authority and Letters of Confirmation are to be requisitioned by Regional Offices and not by Revenue District Offices. Accordingly, all regional directors should now send their requisitions direct to the Accountable Forms Division for sufficient booklets of Letters of Authority and Letters of Confirmation to supply the districts within their jurisdiction. Revenue Districts Officers in turn shall procure blank forms of Letters of Authority and Letters of Confirmation from the Regional Office. A monthly report of Letters of Authority and Letters of Confirmation received from the Accountable Forms Division as well as the distribution thereof to Revenue District Offices should be submitted in four (4) copies to the Central Office distributed as follows: Original Performance Control Div. Duplicate Accountable Forms Division Triplicate Assessment Department Quadruplicate Records Division With respect to blank forms of Letters of Authority pending in the hands of Revenue District Officers as of June 30, 1975, a report thereon should be submitted to the Performance Control Division and a copy furnished the Regional Director.
