June 29, 1977 REVENUE MEMORANDUM ORDER NO. 29-77 SUBJECT : Issuance of Identification Cards on Revised BIR Form Nos. 1.01 and 1.01A to all Internal Revenue Officials and Employees TO : All Internal Revenue Officers, Employees and Others Concerned For the guidance of all concerned and the information of the taxpaying public, revised identification cards will be issued to all officials and employees of the Bureau of Internal Revenue. I. Scope This Revenue Memorandum Order shall cover the manner of issuance of the revised identification cards for all officials and employees of the bureau. II. Purpose Identification cards issued in the past were signed by the Chief, General Services Division. The bureau had been reorganized under Presidential Decree No. 640 and new appointments were extended to all employees where the designations are new so that the old identification cards are no longer responsive to the employees' new designations. In order to remedy this situation and to insure proper and accurate identification of the officials and employees of the bureau, a new procedure for the issuance of identification cards is hereby promulgated for the guidance of all concerned. III. Kind of Cards to be Issued Two kinds of revised identification cards are to be issued, namely: A. BIR Form No. 1.01 to fieldmen B. BIR Form No. 1.01A to non-fieldmen IV. Fieldmen For purposes of this memorandum order, fieldmen shall include the following: A. Revenue Service Chiefs, Assistant Revenue Service Chiefs, Regional Directors, Assistant Regional Directors; B. Chiefs and Assistant Chiefs of the Investigating Divisions in the Central Office; C. Chiefs and Assistant Chiefs of Divisions in the Specific Tax Service; cdt D. Revenue Examiners, Senior Revenue Examiners and Supervising Revenue Examiners assigned in the Divisions mentioned in "B" above and those detailed in other units in the Central Office who are authorized to examine taxpayers' books of accounts; E. Chiefs and Assistant Chiefs of the Assessment, Specific Tax and Collection Branches in Regional Offices; F. Revenue District Officers and Assistant Revenue District Officers; G. Revenue Examiners, Senior Revenue Examiners and Supervising Revenue Examiners in revenue district offices; H. Revenue Examiners, Senior Revenue Examiners and Supervising Revenue Examiners in the Assessment Branches of Regional Offices who perform review and/or office audit functions; I. Revenue Inspectors, Senior Revenue Inspectors, Supervising Revenue Inspectors, Tobacco Inspectors, Senior Tobacco Inspectors and Supervising Tobacco Inspectors; J. District Revenue Collection Supervisors and Revenue Seizure Agents. All other officials and employees of the Bureau who are not included in the above enumeration shall be considered non-fieldmen. V. Description of the Identification Card A. The identification card (BIR Form No. 1.01) is described as follows: 1. It is of the flap type printed on thick white paper 2" x 3". The first flap contains the official letterhead, the picture of the BIR National Office Building, the photo (1" x 1") and the signature of the employee and the identification card number, as shown below: 2. The second flap is also printed on thick white paper which contains the name of the fieldmen, title, description of function, date of issuance and signature of the Commissioner or Deputy Commissioner of Internal Revenue, as the case may be, likewise as shown below: 3. The two printed pieces described above are to be placed inside a plastic leatherette jacket of maroon color and shall be laminated to preclude any tampering. B. The identification card (BIR Form No. 1.01A) is described as follows: 1. It is of the flap type printed on thick white paper 2" x 3". The first flap is exactly the same as the first flap described in V-A-1 above, except with the letter "A" suffixed to the BIR Form Number in this case. 2. The second flap it also made of thick white paper with the following printed thereon: 3. The two printed pieces described above are to be placed inside a plastic leatherette jacket of light blue color and shall be laminated to preclude any tampering. VI. Officials to Sign Identification Cards Identification cards issued to Division Chiefs and other officials with the rank higher than Division Chiefs shall be signed by the Commissioner of Internal Revenue. Those issued to all others shall be signed by the Deputy Commissioner of Internal Revenue. For purposes of this Order, Chiefs of Branches in Regional Offices shall be deemed as having the rank of Division Chiefs. VII. Preparation and Accomplishment of Card A. The General Services Division shall take charge of the preparation and proper accomplishment of identification cards as well as their issuance and release to those concerned. B. The identification card when issued is an accountable property and shall be released under a Memorandum Receipt to be signed by the official or employee to whom it is issued. C. The General Services Division shall collate all the originals of the Memorandum Receipts duly accomplished and transmit the same to the Property Division for file. VIII. Repealing Clause All identification cards issued prior to this Order shall be surrendered to the Property Division. IX.
