Revenue Officer's Audit Report (Applicable BIR Form No. 0500 series). [ ] 5. For self-assessed cases: Tax Return Filed thru the AAB prior to June 30, 2006. [ ] 6. For Accounts Payable or "Due to BIR". Financial Statement and Subsidiary Ledger reflecting the liability to BIR. Affidavit under oath. [ ] 7. Letter Notice. [ ] 8. Copy of court decision and a brief report of its status from the handling office, if applicable. [ ] 9. Payment Form 0605 Evidencing Payment of the 100% Basic Tax. [ ] 10.
