July 19, 1978 REVENUE MEMORANDUM ORDER NO. 26-78 SUBJECT : Adoption of a HISTORY SHEET that shall be Accomplished and Attached to Every Tax Docket TO : Revenue Collection Analysts or Other Officials/Employees Called Upon to Analyze and Take Appropriate Action on Tax Dockets, and Others Concerned For the purpose of having a compact and updated document that shall contain: a. how the tax came about cdt b. kind of tax c. period covered d. sequential narration of all the actions taken on each tax docket, a HISTORY SHEET shall be accomplished and attached to the tax record. The said form shall be accomplished by revenue collection analysts or other officials/employees that shall undertake the preparation of appropriate action/s on a tax docket. The HISTORY SHEET, a copy of which is herewith attached and marked as Annex "A", shall be a standard BIR Form which shall bear the proper BIR Form Number. As appearing on the HISTORY SHEET, any reviewing official/supervisor on any and all actions proposed to be taken, particularly for the collection of the tax, shall sequentially be indicated in the appropriate spaces provided in the said form. The Chief, Property Division shall take steps for the printing and numbering of the said form for distribution to all units concerned for their use as prescribed herein. In the meantime that there are no printed HISTORY SHEETS, every unit of organization shall reproduce the said history sheet for their immediate use in implementation of this Order.
