June 6, 1969 REVENUE MEMORANDUM ORDER NO. 26-69 TO : All Revenue Operations Heads, Regional Directors, Chiefs of Divisions, Revenue District Officers and All Other Concerned. SUBJECT : Accurate Accomplishment of Civil Service Form No. 001 re: Personal Data Sheet To give meaning and substance to effective personnel administration, and to insure the maintenance of an updated revenue personnel record, every revenue official and employee is strictly required to accomplish the enclosed Personal Data Sheet (Civil Service Form No. 001) within five (5) days from receipt of this Memorandum Order duly certified by him/her as to the correctness thereof. Thereafter, such personal data Sheet duly accomplished shall be collated by the Revenue Operations Head, Regional Director, Chief of Division, Revenue District Officer and transmitted the soonest possible to the Chief, Personnel Division who shall cause the preparation of a Master Record based thereon. It is believed that, once a Master Record of all revenue Officials and employees is made, personnel actions will become more systematic, uniform and equitable. aisadc Strict compliance herewith is desired.
