July 6, 1988 REVENUE MEMORANDUM ORDER NO. 25-88 SUBJECT : Amending RMO 28-86 and Prescribing a Personal Data Record Update Form TO : All Revenue Officials and Employees Concerned A. Objectives: This Order is issued to: 1. Distinguish the use of required forms pertinent to the BIR-Personnel Information System (PIS) from those required by the Civil Service Commission. 2. Maintain an updated and accurate data base of all revenue personnel relative to the computerized PIS. casia B. Policies: In accordance to these objectives, 1. The Personal Data Sheet (PDS) provided under RMO 28-86 shall now be known as Personal Data Record (PDR). 2. A Personal Data Record-Update Form is hereby prescribed using the format in Annex A. C. Guidelines & Instructions: 1. The PDR-Update Form shall be filed in duplicate at any time a revenue official or employee needs to update any information such as change of status, additional educational attainments, new eligibilities, promotions and relevant trainings attended outside the BIR. 2. Only the additional and most recent information shall be reflected in the form. 3. The Update Forms shall be accomplished and submitted as follows: a) Those accomplished by personnel in the National Office shall be certified by the head of office who shall forward the same to the Personnel Division. b) Those accomplished by personnel in the revenue region, its branches and units shall be certified by the head of said branches and units who shall forward the same to their respective Administrative Branch. aisa dc c) Those accomplished by personnel in the Revenue District Offices must be certified by the Revenue District Officer who shall forward the same to the Administrative Branch of their respective revenue region. 4. The Administrative Branch in the revenue regions shall retain the duplicate copy for file and transmit the original copy of the Update Form to the Personnel Division. 5. The Personnel Division shall refer all Personal Data Record - Update Forms received to the Management Information and Data Control Systems Division (MIDCSD) for computerization. 6. The MIDCSD shall undertake the computerized processing of the Update Forms after which these forms shall be returned to the Personnel Division for inclusion in respective 201 files. cd i D. Effectivity: This Order shall take effect immediately. Strict compliance is hereby required otherwise the respective heads of offices shall exclude the personnel who fail to comply with these requirements in the list of promotables. BIENVENIDO A.
