August 27, 1985 REVENUE MEMORANDUM ORDER NO. 25-85 SUBJECT : Providing Additional Guidelines Governing the Applications for Sick Leave of Absence TO : All Revenue Officials and Employees in the National Office, Revenue Region No. 4A and Revenue Region No. 4B In addition to the guidelines called for under Revenue Memorandum Order No. 5-82, dated January 20, 1982, all BIR employees of the National Office, Revenue Region No. 4A, and Revenue Region No. 4B, who apply for sick leave of absence for a period of six (6) days or longer, are required to observe the following: 1) All medical certificates issued outside the Bureau must be written on the physician's stationery with his corresponding letterhead. 2) The duration of recommended leave of absence must be stated from the starting date to the last day and not just in general terms of months or weeks. 3) The specific clinical diagnosis must be indicated on the certificate. If an operation was performed, it should likewise be stated. 4) If the absence is one month or more, with subsequent extension, the applicant himself must present the application with supporting papers to the Revenue Physician for purposes of verification, unless otherwise bedridden or incapacitated, physically or mentally, in which case this status must be reflected on the physician's medical certificate. 5) Extension requests must be accompanied by another medical certificate by the physician who recommended the initial leave of absence and the specific reason must be given for such recommendation. aisa dc This Order takes effect immediately.
