June 20, 1988 REVENUE MEMORANDUM ORDER NO. 21-88 SUBJECT : Creation of a Committee on Disposal of Accountable Forms in the National Office TO : All Internal Revenue Accountable Officers, Employees and Others Concerned In order to have a progressive and systematic disposal of all obsolete, damaged, spoiled, cancelled revenue strip stamps and other specialized accountable forms in the custody of the Accountable Forms Division and other revenue accountable officers assigned in the National Office of the Bureau of Internal Revenue, there is hereby created a Committee on the Disposal of Accountable Forms composed of: Chairman Head Revenue Executive Assistant (Inspection) or his duly authorized representative Member Head Revenue Executive Assistant (Financial Management) or his duly authorized representative Member Head Revenue Executive Assistant (Administrative) or his duly authorized representative Member-Witness Resident Auditor, Bureau of Internal Revenue, or his duly authorized representative It shall be the function of the Committee to appraise and conduct an inventory of all accountable forms for disposal referred to it by the Chief, Accountable Forms Division and by other revenue accountable officers assigned in the national Office. Upon the approval by the Commissioner of Internal Revenue, the accountable forms shall be submitted to the Chairman, Commission on Audit or his duly authorized representative for inspection and subsequent destruction in accordance with Section 63 of the National Accounting and Auditing Manual. cdt The Committee shall prepare and submit a report and certificate of destruction to the Commissioner of Internal Revenue after completion of every destruction by burning of particular items or bulk of accountable forms stated in the authority granted by the Commissioner. The Committee shall convene and discharge its functions whenever the necessity therefor exists. All other orders which are inconsistent herewith, are hereby superseded.
