March 4, 2003 REVENUE MEMORANDUM ORDER NO. 18-03 SUBJECT : Revised Access Request Forms TO : All Internal Revenue Officials and Employees Concerned I. OBJECTIVE This Order reformats and prescribes the use of the following numbered Access Request forms in compliance with Revenue Administrative Order No. 2-2002, dated June 14, 2002 delegating to the Chief, Security Management Division the authority to approve the same: BIR Form No. 0033 Special Access Request Form Annex A BIR Form No. 0034 Extension of Special Access Request Form Annex B BIR Form No. 0035 Regular Access Request Form Annex C BIR Form No. 0037 Revocation of Access Request Form Annex D II. REPEALING CLAUSE The reformatted forms prescribed in and made an integral part of this Order replace the old forms originally prescribed in Revenue Memorandum Order No. 2-99 dated November 27, 1998.
