February 9, 1989 REVENUE MEMORANDUM ORDER NO. 09-89 SUBJECT : Prescribing the Report Form for Quarterly VAT Collections and Monthly Submission Deadline TO : All Revenue Regional Directors and Revenue Officers 1. Purpose To ensure prompt and accurate monthly reporting of quarterly value-added tax collections, it is necessary to prescribe a standard and uniform report form and submission deadlines. 2. Requirements a. Form - The report must show: (1) group of taxpayers by filing code; (2) number of registered VAT taxpayers; (3) non-filers; (4) filers; and (5) amount of collections for the quarter. b. Who shall make the report - Every Revenue District Officer shall accomplish the VAT Collection Report (VCR) every month for his respective district office (See Annex A). The Regional Director shall see to it that the reports of the RDO's are made on time and submitted to him for his preparation of the monthly VAT Summary Collection Report (VSCR) (See Annex B). In correcting an error in a previous report, the adjustments must be made in the same form and it must be indicated that such is an amended report. c. Deadline for submission . - The monthly VCR shall be submitted by each RDO to his Regional Director not later than the 25th day of the month in which the quarterly payment is required to be made. Based on the VCRs, the Regional Director shall prepare and submit the monthly VSCR to the VAT Division, but not later than the last day of the month. acd In order not to delay the monthly evaluation of the VAT performance, the Regional Director is required to give an advance report by means of telephone, telegram, or radio communication.
