January 11, 1974 REVENUE MEMORANDUM ORDER NO. 05-74 SUBJECT : Accomplishment of Revised BIR Form No. 1.50 (Partial Leave Form) TO : All Revenue Officers and Employees Concerned A new sense of values has been injected into the main stream of the government bureaucracy under the New Society. Presidential Decree No. 6 dated September 27, 1972 was promulgated to give meaning and substance to these new directions in the matter of the desirable official deportment and conduct of all functionaries in the government service. Notwithstanding the substantial progress achieved under the auspices of the New Society, there are still certain segments in the government service particularly in the Bureau of Internal Revenue who have persisted in maintaining the old discredited ways of misconduct such as to mention a few, coming to the office late, loafing, shopping during office hours, prolonged coffee breaks and absenteeism. If we are to maintain the tempo of progress especially in the improvement of government official behavior, it is necessary that regulations be imposed for observance by all concerned. Accordingly, it is hereby prescribed that effective immediately, all revenue officers and employees with the exception of Department Heads, Assistant Department Heads, Division Chiefs, Assistant Division Chiefs, Regional Directors, Assistant Regional Directors, Chiefs of Branches, Assistant Chiefs of Branches, Revenue District Officers and Assistant Revenue District Officers shall accomplish the Revised BIR Form No. 1.50 (Partial Leave Form), a sample copy of which is enclosed for reference, in triplicate whenever they leave the office premises either on personal or official business. The original copy of the said form shall be retained in the office concerned. The duplicate copy shall be surrendered to a security officer to be designated by the Commissioner of Internal Revenue upon the recommendation of the Regional Director for the regional and the Deputy Commissioner of Internal Revenue (Administration) for the National Office. The triplicate copy shall be retained by the employee. The partial leave form shall be taken into consideration in the preparation of the Summary Monthly Attendance Report and as one of the main factors in the evaluation of the official performance of an employee. All concerned are strictly enjoined to observe the provisions of this Order. Any violation of this order shall be a ground for disciplinary action under Presidential Decree No. 6 dated September 27, 1972. This Order takes effect immediately. MISAEL P.
