January 10, 1968 REVENUE MEMORANDUM ORDER NO. 05-68 SUBJECT : Notice of Loss of Accountable Forms Effective immediately, notices regarding the loss of accountable forms shall be issued by the Department or Division which directly exercises jurisdiction and supervision over the activities of the personnel under whose official custody and for whose account said accountable forms have been issued. This Order is being issued with the end in view to forestall double issuance of revenue memorandum circulars by two divisions under different Departments, which circulars bear on exactly the same subject matter.
