January 15, 1990 REVENUE MEMORANDUM ORDER NO. 04-90 (superseded by RMO 9-95) SUBJECT : Prescribing the Use of the Collection Officer's Monthly Statistical Report of Revenue Collection (BIR Form No. 12.08), Consolidated Monthly Statistical Report of Revenue Collections (BIR Form No. 12.09) and Summary of Monthly Statistical Report of Revenue Collections (BIR Form No. 12.09s) TO : All Internal Revenue Officers and Others Concerned To further improve the reporting of collections, the prescribed BIR Form No. 12.08, Monthly Statistical Report of Revenue Collections and BIR Form No. 12.09, Consolidated Monthly Statistical Report of Revenue Collections under RMO 32-84, as amended by RMOs 4-88 and 19-89 are hereby revised and superseded by the following set of collection reports: acd A. BIR Form No. 12.08 - Collection Officer's Statistical Report of Revenue Collections to be prepared by each Collection Officer. B. BIR Form No. 12.09 - Consolidated Monthly Statistical Report of Revenue Collections to be prepared by each Revenue District Officer based on BIR Form No. 12.08 (Annex B). acd C. BIR Form No. 12.09S - Summary of Monthly Statistical Report of Revenue Collections (by manner of payment and major kind of tax) to be prepared by each Revenue District Officer based on BIR Form No. 12.09. The manner and time of submission of the collection reports shall be within five (5) working days from the end of each month for BIR Form No. 12.08 and within ten (10) working days from the end of each month for BIR Form No. 12.09 and 12.09S.
