January 11, 1995 REVENUE MEMORANDUM ORDER NO. 03-95 SUBJECT : Enforcement and Compliance with Revenue Regulations on the Wearing of Official BIR Identification Cards TO : All Revenue Officials and Employees I. PURPOSE It has been observed that many revenue personnel have not been wearing their official BIR Identification cards regularly in violation of Revenue Memorandum Order No. 25-83 dated July 23, 1983. Therefore, to effectively enforce and ensure compliance therewith this Order is hereby promulgated. II. GUIDELINE IN THE ENFORCEMENT AND COMPLIANCE IN THE WEARING OF OFFICIAL BIR ID A. For security and proper identification purposes, all revenue officials and employees including casuals and contractuals must wear their new official ID cards upon entering and while within BIR office premises ( i.e ., the BIR National Office Building, all Regional and District Offices). B. In the event that the new ID cards have not been received, the concerned employees are hereby instructed to wear their old issued ID cards in the meantime. Wearing of unauthorized duplicated ID cards is prohibited and should be confiscated on the spot by any BIR official and shall be reported to the Chief, Internal Security Division for appropriate disciplinary action. D. Employees, who failed to bring their I.D. cards, should enter through the Visitor's Entrance (NDB) and must sign there names in the Lobby Guard's Log Book, indicated the following: complete name position office/place of assignment destination purpose, and signature The Lobby Guard shall forward to the Chief, Internal Security Division, the list of personnel who failed to wear their ID cards on the first hour of the following day. In the regional and district offices, employees who failed to bring their ID cards must indicate the same information in the logbook provided by the Administrative Officer, who must forward the same to the Chief, Internal Security Division on the first hour of the following day. The Chief, Internal Security Division, in turn, shall send a show cause letter to the personnel concerned, to explain the failure, thru the head of the respective office, and to take the necessary action therefor, if necessary. E. In case of loss of an official ID card, the following must be observed and complied with strictly: 1. Within the National Office Building and premises, the loss must be reported immediately to the Lobby Guards for records purposes. The concerned employee must submit an Affidavit of Loss to the Chief, Internal Security Division, thru the Assistant Commissioner, Intelligence and Investigation Service, within forty-eight (48) hours from the date of loss. 2. Within any of the Bureau's Regional Offices or District Offices, same must be reported within forty-eight (48) hours from the date of loss to their respective Regional Director or District Officer, who in turn, will submit the official report of said loss to the Chief, Internal Security Division, thru the Assistant Commissioner, Intelligence and Investigation Service, within five (5) days from the date of loss. 3. Outside BIR office premises, the loss must be reported immediately to the Police Headquarters having jurisdiction over the general area where such loss occurred. In connection therewith, a Police Blotter and Affidavit of Loss must be submitted to the Regional Director or RDO concerned, within forty-eight (48) hours, and the Chief, Internal Security Division, thru the Assistant Commissioner Service, within five (5) days from the date of loss. 4. The Chief, Internal Security Division, after verification of the records regarding said loss, shall submit its recommendation to the Chief, Personnel Division for the issuance of a new ID card to replace lost ones, within forty-eight (48) hours from receipt of request. III. ISSUANCE OF NEW ID TO REPLACE LOST ID A. Issuance of a new BIR ID card to replace a lost one shall be made upon the recommendation of the Chief, Internal Security Division. B. The Chief, Personnel Division shall issue a replacement for the lost ID card after fifteen (15) days upon receipt of recommendation from ISD with the following payment value. 1st Request : P250.00 per I.D. 2nd Request : P500.00 per I.D. 3rd & subsequent : P800.00 per I.D. Request IV. PENALTIES Non-compliance with this Memorandum Order shall be penalized under CSC Memorandum Circular No. 30, Series of 1989 for Violation of Reasonable Office Regulation, to wit: 1st offense Reprimand 2nd offense Suspension = 1-30 days 3rd offense Dismissal The Chief, Internal Security Division shall conduct an investigation to determine the penalties for non-compliance of this Memorandum Order. V. REPEALING CLAUSE This Order supersedes RMO No. 25-83 dated July 27, 1983 and all administrative issuances inconsistent therewith. VI. EFFECTIVITY This Order shall take effect on February 16, 1995.
