OBJECTIVES This Order is issued to prescribe the use of the following numbered and reformatted Access and Document Change Notice Request forms: BIR Form No. 0033 Special Access Request Form Annex A BIR Form No. 0034 Extension of Special Access Request Form (new form) Annex B BIR Form No. 0035 Regular Access Request Form Annex C BIR Form No. 0036 Update Security & Access Matrix Request Form Annex D BIR Form No. 0037 Revocation of Access Request Form Annex E BIR Form No. 0038 Document Change Notice Request Form Annex F II. REPEALING CLAUSE The reformatted forms prescribed in this order supersedes the forms included in Part V (Forms) of the Security and Access Kit endorsed in RMO No. 65-98 dated July 1, 1998. III.
