January 3, 1995 REVENUE MEMORANDUM ORDER NO. 02-95 SUBJECT : Documentation Procedures and Additional Guidelines for the Taxpayer Service and Information Program (TSIP) for 1995 TO : All Revenue District Officers and Others Concerned The following guidelines are promulgated in order to assist all Revenue District Offices (RDO) in documenting accomplishments for the TSIP for 1995 as prescribed in RMO No. 65-94: (1) To document the participation in taxpayer seminars/dialogues and meetings with accredited agent banks; and the radio and TV appearances, an Appearance certificate as prescribed in Annex A must be accomplished by the concerned BIR official/employee and signed by both the BIR and any responsible representative from the counterpart group involved in aforementioned activities. The Appearance Certificate together with any news clippings, shall be filed for management audit purposes. For seminars organized by the RDO, the program/schedule of events shall serve as the proof of accomplishment. (2) To document the distribution/posting of information materials, the Administrative Section in the RDO shall maintain a logbook containing the following information: (a) Date Distribution/Posted (b) Description and quantity of the Information Materials Distributed/Posted (c) Place where these are distributed/posted (d) Requesting Office/Personnel, if any (e) BIR Personal in-charge of the distribution/posting (f) Signature of the BIR employee in charge of distribution/posting (3) To document the establishment of the taxpayer assistance/tax forms distribution centers, the administrative section of the RDO shall maintain a logbook containing the following information: (a) Inclusive date(s) the center is maintained (b) Location of center (c) BIR Personal in-charge of the center (4) The level of satisfaction in the taxpayer service counters is measured through the Taxpayer Perception Survey Forms (specifically question 3 and 4) which the Taxpayer Service Counters are distributing to the walk-in taxpayers pursuant to RMO No. 12-94. (5) The pertinent documentation (such as memorandum, report, etc.) shall be compiled for the holding of "special event/activities". These special events/activities can include among others the following: motorcades, essay and quiz shows, sports events, open house, exhibits, photo competitions, and slogan/poster contests. This Order shall apply for the TSIP for 1995.
