January 16, 1991 REVENUE MEMORANDUM ORDER NO. 02-91 SUBJECT : BIR Health Evaluation System TO : All Revenue Officials and Employees I. Objective: To establish a comprehensive health evaluation system in the Bureau. To provide and ensure a healthy working force nationwide. II. Basis: Civil Service Commission Memorandum Circular No. 17, s, 1989. II. Organization: In accordance with the above circular directing the establishment of a comprehensive health evaluation system in every agency of the government, there is hereby created a Health Evaluation Task Force in the Bureau, composed of three (3) teams, headed by the following medical officers: Team I Dr. Erwin C. Orteza Team II Dr. Thelma S. Gonzales Team III Dr. Ernesto A. Sotocua IV. Procedure: Except in the National Office, each month, two (2) teams shall conduct physical examination in accordance with the schedule outlined below - EXAMINATION SCHEDULE Date Assignment Team Team Remaining In the Division Feb/March, 1991 National Office I, II, III I, II, III April Manila III II Valenzuela I May, 1991 Quezon City I, III Makati II June Pampanga III I San Pablo II July Baguio I III CAR II August Legazpi I II Tuguegarao III September Bacolod III I Cebu II October Tacloban I III Iloilo II November Cagayan de Oro I II Zamboanga III December National Office I, II, III I, II, III January, 1992 Butuan I II Cotabato III February Davao II I, III The teams may work in the above assignments from 2 to 4 weeks as may be necessary. After February, 1992, the Teams shall follow the above sequence of assignments starting with Manila Region, until all the employees' health evaluation is completed. The Teams shall coordinate with the heads of offices concerned. In the National Office, they shall draw from their Division staff for assistance in the preparation of notes and reports and other tasks. In the regions, the Regional Directors shall furnish clerical and other administrative support necessary. aisa dc V. Supplies & Materials. - Examination materials and office supplies shall be procured through the Property Division. In the Regions, additional office supplies may be requisitioned from the Chief, Administrative Branch. VI. Reports - The Teams shall submit monthly reports to the Asst. Commissioner (Administrative Service) showing the number of examinations and evaluation completed, as well as the number of personnel who will still be examined in that particular assigned area. Individual medical reports shall form part of the medical record of each BIR employee and shall be maintained by the Medical and Dental Division. VII.
