December 7, 2007 REVENUE MEMORANDUM CIRCULAR NO. 91-07 SUBJECT : Lost One (1) Set of Unissued/Filled-Up BIR Form No. 1954 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of one (1) set of unissued/filled-up BIR Form No. 1954-Certificate Authorizing Registration (CAR) with the hereunder stated serial number, to wit: HcSETI CAR200700051761 The abovementioned form was reported as lost by Ms. Laarni DG. Astillero, Revenue Officer, Revenue District Office No. 53, Revenue Region No. 08, Makati City. Consequently, the entire CAR set bearing the said serial number is hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID.
