December 10, 2004 REVENUE MEMORANDUM CIRCULAR NO. 75-04 SUBJECT : Missing Two (2) Sets of BIR Form No. 2524 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing two (2) sets of Revenue Official Receipt forms (BIR Form No. 2524) with the hereunder stated serial numbers, to wit: ROR200301549999 ROR200301550000 The abovementioned forms were reported missing by Ms. Elsie S. Maza, Administrative Officer II, Revenue District Office No. 83, Talisay City, and has consequently been cancelled. All official transactions involving the use of said form is therefore considered as INVALID. TSIaAc Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
