November 12, 2004 REVENUE MEMORANDUM CIRCULAR NO. 68-04 SUBJECT : Cancellation of Three (3) Sets of BIR Form No. 1954 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the cancellation of three (3) sets of BIR Form No. 1954 Certificate Authorizing Registration with the following serial numbers: CAR200400168810 no duplicate copy CAR200400168870 no duplicate copy CAR200400171264 unnumbered original copy The abovementioned forms were cancelled since the first two had missing duplicate copies while the last one had an unnumbered original copy as per report given by Ms. Nanette M. Canillada, Fiscal Examiner I, Revenue Region No. 09, San Pablo City. All official transactions involving the use of said forms are therefore considered as INVALID.
