September 24, 1974 REVENUE MEMORANDUM CIRCULAR NO. 51-74 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One Pad of Fifty Sets of Revenue Official Receipts (BIR Form 25.24) bearing Serial Nos. 3097001-3097050, inclusive Serial Numbers Quantity 3097001 3097050 one pad of fifty sets Mr. ENRIQUE J. SARABIA, former Acting Collection Agent, Kalibo, Aklan in his affidavit dated June 26, 1973 admitted having misplaced several office forms when he was designated Acting Collection Agent. He found out later that among those misplaced was one pad of Revenue Official Receipts with Serial Nos. 3097001-3097050. Likewise in the 2nd Indorsement dated February 15, 1974 of the Provincial Auditor, Kalibo, Aklan, it states that no report of the loss of the above-mentioned official receipts was filed in his office and with this Bureau. It is stated further that the loss, due to misplacement, occurred two years before filing the request for relief of his accountability. Internal revenue officers, employees and others concerned are requested to promptly notify this office in case the above-mentioned receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
