September 26, 1974 REVENUE MEMORANDUM CIRCULAR NO. 50-74 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of two pads of fifty sets per pad, of Central Bank Official Receipts (BIR Form-2) with Serial Nos. 124501-124600, inclusive Serial Number Quantity 124501-124600 two (2) pads of fifty sets per pad, or one hundred sets Mr. AMADO R. BRIAS, Senior Deputy Governor of the Central Bank of the Philippines reported to this Office the loss of two (2) pads or one hundred (100) sets of Central Bank Official Receipts (Form BIR-2) with Serial Nos. 124501-124600 which was issued to the Philippine National Cooperative Bank (under liquidation). Notice of loss of the aforementioned receipts will be published in the Daily Express and the Pilipino Express as undertaken by the Central Bank. It is also stated that in view of the loss, said official receipts are considered cancelled and will not be honored as evidence of payment of national internal revenue taxes. Internal revenue officers, employees and others concerned are requested to promptly notify this office or the Central Bank of the Philippines in case the above-mentioned receipts are found, taking necessary steps and means in the meantime to prevent its improper and fraudulent disposition or use.
