CAR200000079850 1. TVN200000158264 2. CAR200000079859 2. TVN200000158252 3. CAR200000079872 3. TVN200000158288 4. CARZ00000079879 4. TVN200000158265 5. CAR200000079880 5. TVN200000158266 6. CAR200000079893 6. TVN200000158292 The abovementioned Forms which were reported as lost by MRS. CORAZON GARCIA, MRS. CRISTINE ANNE LARROYA and MS. SCARLET FLORENDO of Revenue District No. 3 San Fernando, La Union have therefore been cancelled. All official transactions involving the use of such Forms are therefore considered as INVALID. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this Office, in the event that the aforesaid Forms are found; and to take the necessary measures to prevent the improper or fraudulent disposition or use of said Forms.
