September 25, 1973 REVENUE MEMORANDUM CIRCULAR NO. 45-73 TO : All Internal Revenue Officers, Employees and Others Concerned SUBJECT : Loss of Thirty Seven (37) Pieces of Residence Tax Certificate Class B, BIR Form 9.02 Bearing Serial Numbers 1690514 to 1690550, Inclusive Serial Number Quantity 1690514-1690550 Thirty-seven (37) pieces The above Residence Tax Certificate Class B, Series of 1969 were reported lost by BIR Collection Agent, Mauro P. Lauran, of Atok, Benguet Province on September 20, 1973 to the Accountable Forms Division, however an earlier report was made on January 5, 1973 to the Chief of Police, Baguio City with all the necessary papers showing and proving that Mrs. Petra Mayos, Barrio Captain of Caliking, Atok, Benguet Province lost the above-mentioned residence tax certificates due to fortuitous event. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this Office in case of the above-mentioned Residence Tax Certificates is found, taking necessary steps, in the meantime, to prevent improper or fraudulent disposition or use thereof. aisa dc MISAEL P.
