October 14, 1997 REVENUE MEMORANDUM CIRCULAR NO. 41-97 SUBJECT : Loss of one (1) Original Copy of Land Bank Check No. 982255 (Blank Check) TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of one (1) original copy of Land Bank (Blank Check), to wit: dctai Check Number Quantity 982255 One (1) check The abovementioned Land Bank (blank check) was reported as missing by Thelma Castillo, Cashier II of Regional Office No. 1, Calasiao, Pangasinan and the same has been cancelled and all official transactions, if ever there is any, involving the use thereof are therefore invalidated. Internal Revenue officers, employees and others concerned are requested to promptly notify this Office in case the abovementioned blank check is found, taking the necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
