August 23, 1972 REVENUE MEMORANDUM CIRCULAR NO. 41-72 SUBJECT : Missing Original, Duplicate and Triplicate Copies of Residence Certificate Class "B" (BIR Forms 9.02) No. 168601 TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the missing original, duplicate, and triplicate copies of Residence Certificates "B" (BIR Form 9.02) No. 168601. The above Residence Certificate Class "B" (BIR Form No. 9.02) was discovered lost by collection agent DELFIN P. GARCES, Office of the Collection Agent, Maasin, Southern Leyte. The loss was reported to the Provincial Auditor, Province of Maasin, Southern Leyte, the Revenue District Officer, Maasin, Southern Leyte, the Regional Director, Revenue Region No. 12, Tacloban City and to this office in the 2nd Indorsement of the Acting Assistant Regional Director dated June 19, 1972, attached original with enclosures. Internal Revenue Officers, employees and others concerned are requested to promptly notify this office in case any of the abovementioned Residence Certificate Class "B" (BIR Form 9.02) is found, taking necessary steps in the meantime, to prevent improper or fraudulent use and disposition thereof. aisa dc MISAEL P.
