August 22, 1972 REVENUE MEMORANDUM CIRCULAR NO. 40-72 SUBJECT : Loss of Residence Certificate B (BIR FORMS 9.02), 1972, Bearing Serial Nos. 1442147 to 1442150 TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of Residence Certificates Class "B" (BIR FORMS No. 9.02), 1972, bearing Serial Nos. 1442147 to 1442150. The above Residence Certificates were discovered lost by Mr. RODOLFO R. SORIANO, Revenue Collection Agent of Sta. Cruz, Laguna. The loss was reported to the Provincial Auditor, Sta. Cruz, Laguna, the Revenue District Officer, Revenue District No. 39, San Pablo City, the Revenue Regional Director, Revenue Region No. 9, San Pablo City, the Collection Department, National Office, Manila, and to this Office (Accountable Forms Division), as per Memorandum of the Deputy Commissioner of Internal Revenue (Administration) dated June 20, 1972, original with enclosures. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this Office in case any of the above-mentioned Residence Certificates B (BIR FORMS 9.02), 1972, bearing Serial Nos. 1442147 to 1442150 is found, taking necessary steps in the meantime, to prevent improper or fraudulent use and disposition thereof. aisa dc MISAEL P.
