October 11, 1962 REVENUE MEMORANDUM CIRCULAR NO. 40-62 SUBJECT : Loss of 2 Pads Revenue Official Receipts (BIR Form No. 25.24) TO : All Revenue Officers and Employees and Others Concerned Notice is hereby given of the loss of 2 pads, Revenue Official Receipts, B.I.R. Form No. 25.24, to wit: Serial Numbers Quantity B-1541751-B-1541650 100 Receipts The above Official Receipts were included among the Receipts shipped to the Collection Agent, Tangcal-Magsaysay, Lanao del Norte on December 27, 1961, but said Official reported that he did not receive them. Presumably, they were lost in transit. Internal Revenue Officers and others concerned should promptly notify this Office in case any of these Official Receipts is found taking necessary steps in the meantime to prevent improper or fraudulent disposition or use thereof.
