August 22, 1972 REVENUE MEMORANDUM CIRCULAR NO. 39-72 SUBJECT : Loss of One (1) Unused Booklet or Fifty (50) Pieces of Revenue Official Receipts (BIR Form No. 25.24) Bearing Serial Nos. 5349601 to 5349650-E TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of fifty (50) pieces of Revenue Official Receipts (BIR Form No. 25.24), to wit: SERIAL NUMBERS QUANTITY 5349601-5349650-E 50 receipts The above Revenue Official Receipts were discovered lost by Mr. RAFAEL ZARA, Revenue Collection Agent of Lipa City. The loss was reported to the Revenue District Officer, Revenue District No. 40, Hilltop, Batangas City, the Revenue Operations Head (Collection Department) National Office BIR, Manila and to this Office (Accountable Forms Division) as per memorandum of the Deputy Commissioner of Internal Revenue (Administration), dated June 23, 1972, original with enclosures attached. Internal Revenue Officers, Employees and others Concerned are requested to promptly notify this Office in case any of the above-mentioned Revenue Official Receipts (BIR Form 25.24) is found, taking necessary steps in the meantime, to prevent improper or fraudulent use and disposition thereof. aisa dc MISAEL P.
