June 19, 1974 REVENUE MEMORANDUM CIRCULAR NO. 38-74 TO : All Internal Revenue Officers, Employees, Provincial/City Treasurers and others concerned SUBJECT : Loss of three (3) pads or one hundred fifty pieces (150) of Residence Tax Certificates Class A (BIR Form No. 9.01) Series of 1974 bearing Serial Numbers 4258251 to 4258400, inclusive Serial Number Quantity 4258251-4258400 one hundred fifty (150) pieces The above-mentioned Residence Tax Certificates Class A were reported to the Commissioner of Internal Revenue by GERVACIO T. CANDA, Administrative Officer II and Officer-In-Charge, City Treasurer's Office, Davao City to have been lost while in the custody of the Philippine Airlines, at the Consolidated Terminals, Inc., Legaspi St., Davao City. That Mssrs. ALBERTO A. ALAMA and EUGENIO PATICAN JR., Assistant Administrative Officer and Deputy and Clerk, respectively, verified the contents of the boxes shipped to the City Treasurer and found out that three pads were lacking. That proper claim had been made by the Officer-In-Charge with the Philippine Airlines. Internal revenue officers, employees, Provincial and City Treasurers and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Official Receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
