September 12 , 2001 REVENUE MEMORANDUM CIRCULAR NO. 37-01 SUBJECT : Loss of Three (3) pieces consisting of Unused Original, Duplicate, Triplicate Copies of BIR Form No. 1954 Certificate Authorizing Registration (Transactions Involving Real and Personal Property) TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of ONE (1) ORIGINAL, ONE (1) DUPLICATE, ONE (1) TRIPLICATE copies of unused BIR Form No. 1954 Certificate Authorizing Registration (Transactions involving Real and Personal Property) with Serial Number: CAR199900296376 The abovementioned Forms were reported as lost by MS. CRISTETA A. AGUILAR of Revenue District No. 40 Cubao, Quezon City and have therefore been cancelled. All official transactions involving the use of such Forms are therefore considered as INVALID. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this Office, in the event that the aforesaid Forms are found; and to take the necessary measures to prevent the improper or fraudulent disposition or use of said Forms.
