October 1, 1975 REVENUE MEMORANDUM CIRCULAR NO. 34-75 TO : All Internal Revenue Officers, Employees and Others Concerned SUBJECT : Loss of Two (2) Pads of Tax Payment Acceptance Order (Form CB RCO 10.01), Bearing Serial Nos. 903501 to 903600 Inclusive FORM SERIAL NO. QUANTITY CB RCO 10.01 903501-903600 One Hundred (100) Sets The above-mentioned Tax Payment Acceptance Order Forms were reported lost while in transit on September 10, 1975, by Mr. BENITO E. CALDERON, Revenue District Coordinator of Revenue District No. 58, Dumaguete City. Internal Revenue Officers, employees and others concerned, are requested to immediately notify this Office and/or any of the Revenue Regional Directors or Revenue District Officers regarding any information on this matter and to take the necessary steps to prevent the improper and fraudulent disposition or use thereof.
